Draft a request to reschedule an appointment

A good rescheduling message names the original appointment, offers two or three real alternatives and acknowledges any notice requirement. The assistant drafts it. You send it and wait for the provider to confirm.

A text to start with

I need to reschedule a dental cleaning originally set for [date] at [time] with [provider name]. Their policy asks for 48 hours' notice and I'm giving about 72. I'm free on [date] after 2 p.m. or any time on [date]. I'd like to sound apologetic without over-explaining. Please draft a short email.

What to check in the answer

Read the draft as if you were the person receiving it. The appointment details should be right, the alternatives should match your actual availability, and the tone should sound like how you normally write to this office. Nothing in the draft books a new time, because that happens only when the provider replies. If it gives a reason you never supplied or promises something you can't do, edit those lines before sending.

What to do next

Change any wording that doesn't sound like you, then send the message by whatever channel the provider prefers, usually email or a patient portal. Treat the new time as booked only after you receive a confirmation. If you want a different tone, say so and ask for a revision.
