Prepare a transport note for a care appointment

A driver needs the time, where to arrive, where to be picked up, any access need and who to call. Write only what the provider or the person has confirmed. Leave out the reason for the visit unless the person has chosen to share it with the driver.

A text to start with

I am arranging a ride to an appointment for [person], who agreed I can share logistics with [driver]. Confirmed details: appointment at [time and date] at [location], arrival instruction from the provider is [wording], pickup point is [place], the access need the person asked me to mention is [need], contact is [name and number]. Write a brief ride note using only these details. Do not mention the reason for the visit.

Consent and what the driver needs to know

Check that the person agreed to share the note with this driver. Include enough location detail for the driver to reach the right entrance; a department name may be needed for wayfinding. Leave out unrelated personal details and the reason for the visit. Verify appointment and arrival times against the provider's direct confirmation. State access arrangements in the practical terms the traveler agreed to share. If a location or pickup time remains uncertain, label it clearly and name who will confirm it.

Ask another agent to check the result

Compare the ride note with the supplied appointment confirmation and the traveler's sharing preferences. Flag mismatched dates, times or locations, missing pickup details and personal information unnecessary for the ride. Preserve the location and access information the driver needs. List anything that still requires confirmation.

Confirm before the day

Ask the traveler to review the note. Check appointment details against the provider's direct confirmation and verify the address or entrance instructions. Agree who contacts whom if the ride is late or the appointment changes. Send the current version to the driver and make any correction explicit, so an older pickup time is not used.
