Organize evidence for a performance review

Use the criteria in your employer's review form to organize examples from your work. Give each criterion a separate entry, link the strongest evidence and record any missing detail. Check the review period before choosing a result from an older project.

A text to start with

My review at [employer] uses these criteria: [paste criteria text]. Here are my work examples and links: [examples]. Build a table with each criterion, the best supporting example, an evidence link and any question I still need to answer. Leave a criterion empty when nothing I gave you fits it, and do not stretch an example past what it shows.

Example request. Change the details to fit your situation.

Criteria and evidence table

Review period: [ ] Criteria source (document or page): [ ] Criterion: [ ] Supporting example: [ ] Evidence link: [ ] How the example fits this criterion: [ ] Unanswered question: [ ] Criteria with no evidence: [ ] People to ask for missing examples: [ ] Repeat the item block as needed.

Gaps and overreach

Compare each example with the exact criterion. Attending planning meetings might show participation, but a claim about improving the process needs evidence of what you changed and the resulting effect. Confirm that a team result includes your personal contribution. Leave unsupported criteria open while you look for another example or discuss the gap. Test evidence links with the access available to the intended reviewer, and follow the employer's rules for sharing documents.

Ask another agent to check the result

List every criterion in my table that has no example. Then find claims that say more than the evidence shows, quoting the claim and naming the link that fails to support it. Do not suggest accomplishments I have not described.

Fill the empty rows

For each gap, check whether a useful example exists in your work notes or ask someone who observed the work. If you cannot support the criterion, state that plainly and prepare a question about development priorities. Move the strongest verified examples into your self-review and keep the evidence map for the conversation.
